Telehealth Informed Consent

Salt & Serum Wellness offers care both in person and by telehealth (secure video or other approved technology). This notice explains what telehealth involves and asks for your informed consent before a virtual visit.

1. What Telehealth Is

Telehealth is the use of secure electronic communications to deliver health care when you and your provider are in different locations. It may include assessment, consultation, treatment planning, patient education, and follow-up.

2. The Same Standard of Care

Telehealth visits meet the same standard of care as in-person visits. Your provider will decide whether your condition can be appropriately evaluated by telehealth and may require an in-person visit, laboratory testing, or referral. Telehealth is not appropriate for every concern.

3. Provider Licensure

Telehealth visits are conducted by Florida-licensed Advanced Practice Registered Nurses (APRNs) practicing within their scope of practice and in accordance with Florida law and applicable physician-collaboration requirements.

4. Potential Benefits

  • Improved access to care and reduced travel.
  • More convenient scheduling and continuity of care.
  • The ability to involve your provider from a location that is comfortable for you.

5. Potential Risks & Limitations

  • Technology may fail or be interrupted before or during a visit.
  • A remote evaluation has limits — your provider cannot physically examine you, and some conditions require an in-person visit, labs, or imaging.
  • Information transmitted (for example, a video image) may be insufficient for sound clinical decisions, requiring an in-person follow-up.
  • In rare cases, security protocols could fail, causing a breach of privacy.

6. Privacy & Security

We use technology intended to be secure and, where required, HIPAA-compliant. Please join your visit from a private location. Standard email and text messaging are not fully secure. How we handle your information is described in our Privacy Policy & Notice of Privacy Practices.

7. Medical Records

Telehealth visits are documented to the same record-keeping standard as in-person visits.

8. Prescriptions via Telehealth

Medications are prescribed only when clinically appropriate after an adequate evaluation and consistent with Florida law. Salt & Serum does not prescribe controlled substances via telehealth. Some therapies may involve compounded or non-FDA-approved products; these are explained in our Medical Disclaimer.

9. In an Emergency

Telehealth is not for emergencies. If you believe you have a medical emergency, call 911 or go to the nearest emergency room. At the start of each visit, please tell your provider your physical location so help can be directed to you if needed.

10. Costs

Telehealth visits are self-pay. You have the right to a Good Faith Estimate of expected charges before scheduling.

11. Your Consent & Right to Withdraw

By participating in a telehealth visit, you consent to receive care this way. You may withhold or withdraw your consent at any time and request an in-person visit instead, without affecting your right to future care.
