Privacy Policy & Notice of Privacy Practices

Salt & Serum LLC ("Salt & Serum," "we," "us," or "our") respects your privacy and is committed to protecting the personal and health information you share with us. This Privacy Policy explains what we collect, how we use it, and the choices you have. It also serves as our summary Notice of Privacy Practices for protected health information.

1. Who We Are

2. Information We Collect

Information you give us

When you submit a form, book a service, or contact us, we may collect your name, email address, phone number, location (such as your hotel, rental, or neighborhood), the services you are interested in, and any details you choose to share in a message.

Health information

When you become a patient, we collect health information needed to provide safe, appropriate care — for example, your medical history, medications, lab results, treatment goals, and notes from your visits. This is "protected health information" (PHI) and is handled as described in Section 5.

Payment information

Payments are processed by our third-party payment provider, Square. Your full card number is entered directly with Square and is not stored on our servers. We may retain limited records such as the amount, date, and a receipt reference.

Information collected automatically

Like most websites, ours may receive basic technical data (such as IP address, browser type, and pages visited) through standard server logs and any analytics or web-font services we use. Our site loads fonts from Google Fonts, which may receive your IP address as part of delivering those fonts.

3. How We Use Your Information

  • To respond to your inquiries and schedule and provide services;
  • To deliver clinical care, order labs, and follow up on your treatment;
  • To process payments and send receipts;
  • To send service-related communications (such as appointment coordination);
  • To operate, maintain, and improve our website and practice; and
  • To comply with legal, regulatory, and professional obligations.

4. Text Messaging & Communications

If you provide your phone number, you consent to be contacted by phone, text, or email about your care and requests. Message and data rates may apply. You can opt out of non-essential messages at any time by replying STOP or contacting us.

5. Your Health Information & HIPAA

As a healthcare provider, we are required to protect the privacy of your protected health information (PHI). We use and disclose PHI primarily for the following purposes:

  • Treatment — to provide and coordinate your care;
  • Payment — to bill and collect for services provided; and
  • Healthcare operations — to run our practice safely and lawfully.

Other uses and disclosures generally require your written authorization, except where permitted or required by law. You have rights regarding your PHI, including the right to access and request a copy of your records, request corrections, request restrictions on certain uses, request confidential communications, receive an accounting of certain disclosures, and receive a paper copy of our notice. To exercise these rights or file a privacy complaint, contact us at Saltserum@gmail.com. You will not be retaliated against for filing a complaint, and you may also complain to the U.S. Department of Health and Human Services.

This section is a plain-language summary. A complete Notice of Privacy Practices, reviewed by counsel, should be provided to and acknowledged by patients at intake.

6. How We Share Information

We do not sell your personal information. We share it only as needed:

  • With service providers who help us operate — for example, Square (payments) and our web form provider (which delivers your form submissions to us by email);
  • With other healthcare providers or your home provider when coordinating your care, with your consent;
  • When required by law, subpoena, or to protect health and safety; and
  • In connection with a business transfer (such as a merger or sale), subject to applicable law.

7. Data Retention & Security

We keep information for as long as needed to provide services, meet medical-record retention requirements under Florida law, and comply with legal obligations. We use reasonable administrative, technical, and physical safeguards to protect your information; however, no method of transmission or storage is completely secure.

8. Your Choices & Rights

You may request to review, update, or delete personal information we hold about you, subject to limits that apply to medical records and legal requirements. 9. Children's Privacy

Our website and services are intended for adults (18+). We do not knowingly collect personal information from children without appropriate consent.

10. Third-Party Links

Our site may link to third-party websites or services. We are not responsible for their privacy practices, and we encourage you to review their policies.

We may update this Privacy Policy from time to time. The "Last updated" date above reflects the most recent revision. Material changes will be posted on this page.
