You have the right to be treated with courtesy and respect, with appreciation of your individual dignity, and with protection of your need for privacy. You have the right to impartial access to treatment regardless of race, national origin, religion, disability, or source of payment.
You have the right to know the name, function, and professional credentials of the providers caring for you, and to receive information about your diagnosis, planned course of treatment, alternatives, risks, and prognosis in terms you can understand (unless it is medically inadvisable to share such information). Care at Salt & Serum is provided by Florida-licensed Advanced Practice Registered Nurses (APRNs) practicing in accordance with Florida law and applicable physician-collaboration requirements. You may verify the license of any Florida health care professional through the Florida Department of Health’s license verification portal at mqa-internet.doh.state.fl.us.
You have the right to participate in decisions about your care, to give informed consent before treatment, and to refuse any treatment except as otherwise provided by law — and to be told the medical consequences of refusing. You have the right to know whether a treatment is experimental and to consent to or refuse participation.
You have the right to a reasonable response to your requests for care, and to be told if a referral or transfer to another provider is needed, why, and what alternatives exist.
Salt & Serum is a self-pay (cash-pay) practice and does not bill insurance. You have the right to a reasonably clear and understandable, itemized bill and, upon request, to have the charges explained. Under the federal No Surprises Act, uninsured and self-pay patients also have the right to a Good Faith Estimate of expected charges — see our Good Faith Estimate notice.
You have the right to confidential treatment of your medical records and communications. How we use and protect your health information is described in our Privacy Policy & Notice of Privacy Practices.

If you feel your rights have not been respected, please tell us first — contact us at Saltserum@gmail.com or (786) 650-7240 so we can try to resolve your concern. You also have the right to express grievances to the appropriate state licensing agency. Complaints about a Florida health care practitioner may be filed with the Florida Department of Health, Division of Medical Quality Assurance, through its online complaint portal at online-complaint.mqa.flhealthsource.gov. Florida law requires such complaints to be submitted in writing and signed.
